CHANS Receives National 5-Star Rating

CHANS Home Health & Hospice recently received a 5-star rating from the Centers for Medicare & Medicaid Services (CMS). The rating is based on data collected in a nationally standardized survey that asks patients about their recent experiences with CHANS. CHANS 5-star rating is published on Home Health Compare, an online comparison tool that allows consumers to make more informed decisions when choosing a home health provider.

CMS began publicizing the new Patient Survey Star Ratings in January 2016. Consumers are now able to view and compare patient star ratings on the following five topics: Care of Patients, Communication between Providers and Patients, Specific Care Issues, Overall Rating of Care Provided by the Home Health Agency, and Survey Summary. CHANS scored over 86% in each of the reportable areas, warranting the 5-star, highest available rating.

Since 1947, CHANS Home Health & Hospice has worked directly with patients, families, and providers to deliver and coordinate nursing, rehabilitation, hospice, palliative, pediatric, and private duty care, offering home based care for short term and long term needs. Since 2001, CHANS has maintained Joint Commission accreditation and has received three successive deficiency-free surveys from the Department of Health and Human Services (DHHS).
